To get things started, I will need the following information (or as much as you can provide at this time):

· Your name and email/phone number/mailing address

· Your event planner’s name (if applicable) and email/phone number/mailing address

· Type of event (wedding ceremony, cocktail hour, reception, party, dinner, etc.)

· Date of event

· Times

► For a wedding ceremony, please indicate the start-time of the ceremony. Twenty minutes of prelude music prior to the start time is included in a standard wedding ceremony.

► For receptions, parties and other events, please indicate the start and end times for the musical entertainment.

· Venue

► Name of the event venue

► Contact person and phone/email at the event venue (usually the event/banquet manager)

► Location of event venue. I will need the address, city, state as well as the specific location at the facility, as some venues have multiple buildings and event rooms.

· Number and type of musicians (we will discuss options, but let me know what you have in mind)

· Any special situations regarding location (outdoors, piano on site, etc.)
